STEPS Network Meeting January 19, 2005

Purpose: The South Texas Exploration and Production Safety Network, commonly known as the STEPS Network, promotes safety, health and environmental improvement in the exploration and production of oil and gas in South Texas . The STEPS Network fosters a work environment that relies upon open communication and trust.
Marianne McGee, OSHA Compliance Assistance Specialist, Corpus Christi Area Office
- Five work related fatalities reported to the CC Area Office since October 20, 2004 . Two were oil and gas related.
- Welder crushed by the monkey board
- Employee crushed between casing pipe and monkey board
- Employee crushed by a large piece of dislodged concrete.
- Employee crushed by falling sign
- Employee struck by rim segment of split rim wheel
Sharing of incidents:
A network member shared that while an employee was working aloft, the top drive started up. This was a close call. Changes to the procedures have been implemented to require the top drive be locked out prior to the employee going aloft.
A network member shared an incident of an employee crushed. Emphasized the importance of inspecting tong guide.
Serena Starkey TWCC, Worker Safety and Health Division and Ron Tom, TWCC, OSHA Consultation Services discussed the services free of charge to employers in Texas . Free resources and training materials on the website and onsite consultation is available to employers.
Warren Hubler, H&P Drilling: Common incidents and common practices
- Implemented the Good Catch program to identify objects that can potentially be dropped and injury an employee.
- Discussed motor vehicle incidents.
- Utilize STOP the job events where an employee stops a job that presents a potential safety or environmental hazard.

Brad Livingston: In a Split Second
- September 20, 1991 working as welder's helper. Repairing a pinhole leak on two tanks and did not thoroughly evaluate the level of the tanks. One tank and then the other exploded. The welder was fatally injured and Brad was burned over 63% of his body.
- What did I do to my family because I took a shortcut on the job?
- Just a second is all it takes to stop an unsafe job and save a life.
